We want to make sure every patient and family member has the best and safest experience possible and we strive to achieve this by encouraging open lines of communication and by following and implementing safety measures that ensure the highest level of health care.

The work we have done to decrease unplanned readmissions, reduce infections and limit medication errors are just a few examples of the many ongoing projects to which we owe our success and set the standard for other medical centers worldwide.

FallsWheel

Reducing Patient Falls

Fall and fall-injury prevention is a priority across all health care settings at Ohio State's Wexner Medical Center. Research shows that having all patient care team members involved in fall prevention strategies has the greatest impact on preventing falls and fall-related injuries.

Patients in the hospital are often connected to equipment or taking medicines that make them unsteady. We encourage patients to ask us for help if they need help to get out of bed and have put strategies in place to ensure a patient's safety and to reduce falls.

Working together, patients, families and health care professionals from our medical center developed a tool, the Falls Wheel, to assess a patient's fall risk. Using this fall-injury prevention strategy has allowed us to reduce hospital falls by 20 percent.

Be a Partner in Your Care

We encourage our patients and their family members to communicate with us about any concerns they have – whether it's a new symptom or if while in one of our hospitals they feel worse than they did the day before.

Clean hands are a top priority and the best way to prevent the spread of infection. We remind our patients and their visitors to keep their hands clean. Patients also have the right to ask health care team members to wash their hands using alcohol-based hand rub or to wear gloves before touching them.

We encourage our patients to bring a complete and updated list of medicines, including over-the-counter products, vitamins and herbal supplements they are taking to all appointments and admissions. This helps our care team ensure that we don't prescribe any medicines that could negatively affect you.
