Steps to Bariatric Surgery

  • Attend a free, mandatory Information Session
    • Learn about the benefits of having bariatric surgery at Ohio State
    • Meet Ohio State's Bariatric Surgery team
    • Learn more about the surgery options at Ohio State
    • Ask questions
    • Hear success stories from bariatric surgery patients
    • Receive an application packet for weight-loss surgery
  • Call your insurance provider to check if:
    • Bariatric surgery is a Covered Benefit (If exclusion – not covered)
    • The Ohio State University Hospitals and surgeons, Dr. Bradley Needleman and Dr. Dean Mikami, are in your Provider Network
  • Complete the application packet
    • You will be given a packet to complete and it is important to make sure you complete with as much information as possible for us to use when submitting to your insurance company.
  • Call 614-366-6675 to schedule evaluation appointment
    • You will be screened over the phone by being asked several questions.  Depending on insurance, and if you meet criteria, you will be scheduled for your initial evaluations.  You are required to have a medical, dietary and psychological evaluation
    • Most appointments are held in Ohio State's Martha Morehouse Tower building on the 4th floor. 
  • Attend evaluation day appointment – please bring insurance cards, completed application and completed insurance form. Evaluation day will include:
    • Medical evaluation
    • Dietary evaluation
    • Discussion of insurance requirements and next steps
    • Lab work
    • Other tests may be recommended
  • Attend Weight Management Program at Ohio State's Medical Center – required by most insurance companies
    • These classes provide diet, exercise and behavior teaching to prepare you to be successful after surgery.
  • Review of your file by medical staff
    • To determine if any additional tests need to be done
    • To determine risks vs. benefits of surgery
    • You will be notified of surgeons' recommendations
  • Submission of Insurance
    • You will be contacted when your file is being prepared to submit to insurance
  • Attend preoperative appointment with the surgeon
    • Surgeon's office will contact you for your preoperative appointment
    • At this meeting your surgery date will be determined
