Payment and Billing

OSU Primary Care Network offices request payment at the time of service. If you are covered by an insurance plan that requires a co-payment, we require this co-pay at the time of service as well.

Insurance Coverage

OSU Primary Care Network offices accept most major insurance plans. To verify that the provider you see participates in your insurance plan, please contact your insurance company. If you are part of an insurance plan, including Medicaid, please bring your insurance card as you will be asked to present it at each visit.

Appointment Cancellations

Should you need to cancel your appointment, we ask that you please notify your Primary Care office within 24 hours of your appointment. This allows us to accommodate other patients more quickly.

Consent to Treatment Form

All patients will be asked to sign a Consent for Treatment Form. Under the laws of Ohio, a minor or someone who is under 18 years of age is required to have a written permission for treatment from a parent or legal guardian before a physician sees the minor. The Primary Care offices have these forms for you.

Referrals

Your insurance company may require a written referral from your physician for treatment by a specialist. Please notify your Primary Care office in advance. A medically urgent referral can be completed at the time of your visit. Please allow us 5 to 7 business days to complete a more “routine” referral.

Emergency After Hours

For medical problems after our office hours, please call your Primary Care office and you will be instructed on how to contact the physician on call. The physician will return your call as soon as possible. If you have a health emergency, please call 911 or go to an emergency department. We encourage you to follow your insurance company’s procedure for emergency care.
